Breast Health Coordinator needed in Sioux Falls, SD

Our client, a 400+ bed facility in Sioux Falls, has an immediate need for a Breast Health Coordinator in their cancer center. This is a brand new position that will report to the Director of the cancer center. This position is responsible for the continuity of care and social support systems for patients and their families throughout their health care experience.

Develops education and training programs for patients, physicians and breast care staff on the appropriate clinical, community requirements and programs
Works with community advocates for the breast care program
Tracks and trends data for the program reporting requirements.
Collaborates with patient/family and prioritizes patient needs, problems and goals. Intervenes and prepares referrals as needed.
Tracks appointments and referrals including support services and communicates with primary care centers re: patient needs.
Applies specialized expertise in Hematology oncology, Radiation Oncology, surgery and diagnosis and treatments to solve problems, generate alternative care, map care plans and delivery
Educate and train staff and patients as needed to achieve outcomes of the program
Evaluates, compares, trends and interprets patient data
Takes ownership of assignments; other duties as assigned or requested.
Communicates and listens effectively with internal and external customers; effectively understands instructions and shares knowledge
Actively involved in community cancer programs and outreach.
Cooperates and interacts with supervisors, peers, other departments, and all customer groups demonstrating our commitment to ?service?
Adheres to policies, procedures and regulations to ensure patient safety and compliance
Excellent salary and relocation available.

Requirements:

Currently licensed as an RN or able to be licensed as an RN in South Dakota.
OCN preferred.
