Oncology Nurse Navigator

Functions as a clinical resource to patients, physicians, and the cancer program for assigned cancer site(s) - Focus will be on Lung, Colo-rectal, and Prostate Cancer Sites. Utilizes the nursing process to provide coordinated patient care by actively collaborating with other members of the healthcare team and case managing patients. Provides direct patient and family education, as well as community outreach, education, and screening.
